Community Service Broker Model:

The Community Service Broker Model assists people with disabilities who qualify for DSPD services and are currently on the DSPD waiting list.

The goal of this model is to link people with disabilities with needed community resources which will strengthen the family and enhance the quality of life.

The Community Service Broker links individuals and families to natural community supports in order to promote family preservation. The Community Service Broker will help direct families and individuals to community resources that are out side the services paid for by DSPD.

NOTE: Community Service Brokering is only available when appropriate for the specific Person and when funding is available..
